If your form doesn’t work after you enter your details and click submit there is probably an issue with your javascript file.
A common problem is your browser will have cached an old version of the QuickCab javascript files, to empty your cache and force the browser to fetch a new version you need to do a hard reload:
Mac: cmd+shift+r
Windows: Ctrl+F5
